This patient, known with rheumathoid arthritis, presented with a volar soft tissue mass of the wrist, confirmed on x-ray and ultrasound. MRI postulated the diagnosis of a flexor tendon rice body tenosynovitis with multiple avascular 'rice grain like' synovial proliferations, hypo-intense on T1 (first image) and slightly hyperintense on T2 (second image) compared to muscle. Post contrast imaging (third image) illustrates the avascualr aspect of the rice bodies and will only show synovial wall enhancement in the context of chronic synovitis.
