Management of simple ovarian cyst

 

  • In women of reproductive age

    • Cyst size ≤ 3 cm

      • Normal physiologic finding

      • May or may not be described in imaging report

      • Does not need follow-up

    • Cyst size > 3 and ≤ 5 cm

      • Should be described in imaging report with statement they are almost certainly benign

      • Do not need follow-up

    • Cyst size > 5 and ≤ 7 cm

      • Should be described in imaging report with statement they are almost certainly benign

      • Yearly US follow-up

    • Cyst size > 7 cm

      • May be difficult to assess completely with US, further imaging with MR or surgical evaluation should be considered

  • In postmenopausal women

    • Cyst size ≤ 1 cm

      • Clinically inconsequential

      • May or may not be described in imaging report

      • Does not need follow-up

    • Cyst size > 1 and ≤ 7 cm

      • Should be described in imaging report with statement they are almost certainly benign

      • Yearly US follow-up, at least initially

        • Continue follow-up annually; decrease frequency of follow-up once stability or decrease in size has been confirmed

        • Cysts in larger end of this range should be followed on regular basis
    • Cyst size > 7 cm

      • Further imaging with MR or surgical evaluation should be considered
